Hello Every One

I have tried the function Given into documentation but i cannot able to create Combo Box into form

I have tries Following function

NewFormField("TestField",1) (I have tried 1 to 6 as second parameter)

SetFormFieldComb(index,2)

is there any wrong , or any other way is there for creating ?

If detailed list is available about which fields we can add using isedquick than it will better.
